I hereby voluntarily consent to the collection, processing, storage and use of personal data by BenFit Nutrition GmbH, as well as to the transfer of such data to all parties necessary for the performance of the contract (e.g. doctor, health insurance company), where it may also be processed and used. A transfer of my data to third parties may only take place with my prior consent.

In particular, I agree to the processing of the following health-related data documenting the course and effectiveness of treatment: e.g. physical measurements such as weight and height, BMI, laboratory values, symptoms of illness, recommendations.

I agree that my data may be used for personal contact (e.g., appointment scheduling, appointment cancellations, appointment reminders, follow-up offers).
